Can you please help me with wording for a party for a 5 year old who is leaving school due to leukemia?

Can you please help me with wording for a party for a 5 year old who is leaving school due to leukemia?
There is a little girl in my son's kindergarten class that is leaving school to go to St. Jude's Hospital for treatment for her leukemia. I am planning to have a simple party to celebrate families because that is what they are learning in school but also as a farewall to a little girl who is leaving. We plan to have milk & cookies, decorate picture frames and place each childs family picture in their frame to take home with them. We also plan to make an extra frame for the little girl with a picture of all her classmates in it. We also plan to give her a webkin (toy). Can you help me with maybe a cute poem or wording for an invitation? The little girls name is Madelyn. I appreciate any help!!
wording for an invitation is what i meant to write.
Does it sound okay if I say . . . . Please come join us as we celebrate family and friends with special thoughts for Madelyn as we wish her a speedy return to our class.

Please come and celebrate with me on my last day here. Your friendship means the world to me and I would like to take all of you with me but since I can't, won't you please join me for some milk and cookies?

or something along those lines. Hope she gets well soon. I will say a prayer for her..

i first off just want to say God bless you for how nice you are! a friend of mine back in high school had leukemia except he was in remission. it is really hard to even start to put how they feel or what they are going through into words. the easiest thing to say is good luck with everything madelyn! my daughter has to go to childrens mercy all the time due to being blind and what you are doing will not only mean the world to madelyn but to her family as well. i dont really know what to say to her in a poem or anything but God bless you for your kind heart! i just want to say thank you!

Dear All,
One of our beautiful children, Madelyn (last name) is leaving us on (enter your date). Good-byes are always hard to say; therefore, our kindergarten class is going to have a small gathreing with food and activities. Everyone is invited to come to let this precious girl know how much she is loved and will be missed after she leaves our class. Madelyn is a member of this great class and her presence will deeply be missed. So, come join us at (place) on (day), (date) at (time) for a(n) (morning, afternoon, or evening - whichever it is) full of fun for all!

R.S.V.P. (optional)

[hope this helps in writing this invitation......my sympathy to all...you are extremely kind-hearted...may madelyn be blessed for a recovery..may you be blessed for your kind heart and thoughts]

I would try to avoid words like farewell and leaving us because it sounds like death honestly. Maybe a bon voyage type party message?

Please join us for an afternoon celebrating family and friends with a special bon voyage for Madelyn as we wish her a happy & healthy return.

Something like that maybe?
EDITED TO ADD: The new wording sounds good!

You are doing a wonderful thing but please do not add the cloud of her condition to the festive nature of the party. It will create an atmosphere that will be counter to your intent and the children will not understand, or worse, they will understand and become worried and concerned for their friend, and for themselves because that is how 5 year olds think. Telling them might even cause them to avoid or ostricize the little girl you want to help. Just have a going away party for the girl who is leaving and wants to say good bye to all her little friends. That is the truth and enough information for the little ones to handle. If they are aware of her condition then it will make sense to them that she needs to go to the hospital. That is more than enough. Let it be the sunny, happy time of good memories that you want. There is pleanty of time for the coming clouds
